• Please read our Adoption Policies  before completing this application.

• We ask that you complete this application thoroughly with as much useful  information as you can provide.

• You do not need to provide an exact address for your references but we do need at least a city and telephone number.

• Any personal information obtained is confidential and is used for identification and verification purposes related to the adoption of a dog from our organization. It will only be viewed by members of our organization who are directly involved in the adoption process.

• Once you submit your application we will contact the references you provide, unless you direct us otherwise.

• Please understand while your motives may be sincere, we rescue a breed of dog that is many times sought for reasons other than a family pet or service dog. If our questions seem intrusive, it is only for the well being of our rescued dogs. These questions allow us to better determine if the dog available for adoption would be best suited for your home and family.
